October 1, 2021 Dear Partner in Caring, November is National Alzheimer’s Disease Awareness Month and National Family Caregivers Month. The Alzheimer’s Association CT chapter has traditionally held Appreciation Luncheons to recognize individuals demonstrating “Excellence in Caregiving.” Again, this year, with the health and safety of our constituents, volunteers, and staff in mind, we will be honoring and celebrating these individuals from a virtual platform. You are invited to nominate two caregivers. Nominees can be professionals or family members who demonstrate unselfish devotion to the complex and courageous task of caring for those with Alzheimer’s disease or a related dementia. We are accepting a maximum of two nominations per family/facility/agency. The Eastern Region online celebration will be on Friday, November 19th. Nominees will be notified about their recognition and provided a zoom link to attend with a guest. Their certificates will be mailed to them after the virtual ceremony. It is our hope that you as the nominator will join in the event. Below you will find a link to the nomination forms. One form is for the Professional caregiver nominee and one for the Family/Friend caregiver nominee. Please provide one paragraph (NO MORE than 100 words) describing your nominee’s outstanding caregiving traits or contributions and submit by October 30th. A Professional Caregiver includes but is not limited to staff from skilled care, assisted living, homecare, adult day, senior centers, hospitals and senior housing. A Family/Friend Caregiver can be selected from support group participants, a family member in your facility, agency, hospital or center or a family or friend caregiver you are aware of in the community.
