Center for Hospice Care Southeast Connecticut is a nonprofit community-based organization that has been serving Eastern Connecticut since 1985. We provide end of life care to anyone, regardless of age, disease, illness or inability to pay. We also provide palliative care for those still battling illness. We use a team approach, involving the patient’s physician and the hospice medical director, as well as nurses, social workers, hospice aides, bereavement counselors, and volunteers. Together, this group provides support, education, and choices for the patient and family.Caring for the whole community. We provide grief support to the entire southeastern Connecticut community at our Community Bereavement Center in Norwich and at other locations throughout the region. These support groups are provided at no cost and are designated for adults, teens and children – anyone who is struggling with the loss of a loved one. Center for Hospice Care Southeast Connecticut is a Medicare and Medicaid certified, community based, nonprofit hospice. Hospice care is reimbursed by Medicare, Medicaid and most private insurers. Center for Hospice Care needs the financial support of the community to continue offering the full range of services that help ease a patient’s symptoms and provide emotional and spiritual support to the patient and their family.
